I assume full responsibility for my child while in attendance or in transport to or from New Beginnings Community Church of Ambler.  By doing so, I release New Beginnings Community Church of Ambler and its agents and employees from any liability for injuries incurred by my child.  I understand that, in the event that medical treatment is required, every effort will be made to contact me.  However, if I cannot be reached, I give my permission to the staff to secure the services of a licensed physician to provide the care necessary, including anesthesia, for my child’s well-being.
